Hanging over your head at the top of the frame (to be comboed with worried-looking eyes) was what I had in mind. A frame actually in contact with the head as well as one on the ground in front of the avatar would be good too.

Speaking of frames, maybe a picture frame that's like...waist-up? So you can layer shirts and stuff, and the bottom of the frame can cover it. Or maybe some cloth draping below the frame so you don't have to worry about legs and hands.
